Bu gibi durumlarda cerrahi yollarla testis içinde sperm aramak gerekir. Testiste sperm üretiminin olduğu biliniyorsa iğneyle testisten sperm çekilebilir. Bu tekniğe TESA denilmektedir. Testiste sperm üretiminin olup olmadığı net olarak bilinmiyorsa mikro-TESE dediğimiz cerrahi yöntemle testisten sperm elde edilebilinilmektedir. Mikro-TESE yöntemi diğer cerrahi yöntemlere göre yüzde 50'den daha fazla sperm bulma şansı vermektedir.
